Print Out Fond du Lac City Dart Tournament Registration Forms here by clicking on the link to the form below and then printing it out.  Turn in completed forms with your Team Sponsor Fee to the Press Box before March 5th, 2010!  If you prefer, give us a call at (920) 929-9735 to make arrangements for us to pick it up.  You can also register online below but you will not be entered into the tournament until your Team Sponsor Fee has been received, NO EXCEPTIONS!  All online registrants will be called to confirm their entry so if you register online and don’t receive a call within 48 hours of registration, please contact us immediately. For the Team Event, players must have played in a FDL league and have an established average or have shot in any JBM Amusements league in order to be eligible! Mixed Trips is open to anyone!
